  • Here’s how it works

    When you complete a healthcare activity, let us know a few details and we’ll send your gift card; it’s that simple. Complete your activity by December 31, 2025, and let us know by January 31, 2026 in order to redeem your reward.

    We are open 8 a.m.–8 p.m. PT Monday through Friday from April 1 through September 30 and 8 a.m.–8 p.m. PT seven days a week from October 1 through March 31 (except certain holidays).

    Visit the Member Portal. Click “Sign In” to register and log in. You can view and attest to your qualifying healthcare activities.

Annual Wellness Visit (AWV)

The Annual Wellness Visit is a planning session with your primary care doctor to discuss how you can improve your health and avoid or reduce the effects of certain conditions.

During the visit, your height, weight, and blood pressure will be taken. The visit will include a complete review of your medical history, family history, and any current health concerns. You might be referred to other screenings or services. The purpose of the wellness visit is to review your overall health and discuss prevention and wellness. You will have dedicated time to discuss your health concerns, fi nd ways to improve your overall health, and create a personalized prevention plan that includes regular screenings and wellness goals. Instead of going to your doctor only when you’re feeling ill, this visit is intended to help you maintain your optimal health.

What to expect

The visit is not the same as a physical exam, although your healthcare provider may do a simple vision test and take your height, weight, and blood pressure. You may also be asked about your eating habits, physical activity, and general well-being. Your doctor will refer you to the appropriate specialists for recommended disease screenings.

Next Steps:

  1. Schedule your AWV by contacting your primary care physician (PCP).
  2. Prepare for your visit and collect a list of your current medications.
  3. Know that your AWV is covered at no cost to you.

Fall Prevention Assessment

Falls are a threat to your health, and they can reduce your ability to remain independent. The good news is, many falls are preventable.

One in four people aged 65 or older falls each year; and many of those falls cause serious injury such as broken bones or head injuries. Most falls occur in or around the home. That means that most falls can be prevented. Join us at one of our Balance Your Body classes, or sign up for a one-on-one fall prevention assessment by phone with one of our health coaches.
